ACL, MCL, LCL: Choosing the Right Knee Brace After a Ligament Injury
Ligament injuries are frustrating in a particular way — you usually know exactly when it happened, you remember the pop or the twist, and then you spend weeks or months trying to get back to whatever it was you were doing before. The brace question comes up pretty early in that process, and it's one where there's a lot of conflicting information out there.
So let's go through what actually matters when you're choosing a brace for a ligament injury — and why the type of ligament involved changes what you're looking for.
Why Ligament Injuries Need More Than Just Compression
When a ligament is sprained or torn, the joint loses some of its passive stability — the structural check that normally prevents excessive or harmful movement. Depending on which ligament is affected and how severely, the knee may feel unstable, buckle under load, or simply not feel trustworthy anymore.
A compression sleeve gives you proprioceptive feedback and mild support, which is useful early on. But for moderate-to-severe ligament involvement, you need a brace that actually limits the specific movements the injured ligament is supposed to control. That's where hinged and functional ligament braces come in.
ACL Injuries
The ACL controls anterior translation and rotational stability — meaning it prevents the tibia from sliding forward on the femur and from rotating excessively. When it's injured, the knee tends to feel unstable in pivoting and cutting movements, which is why ACL injuries hit athletes in rotational sports so hard.
For ACL injuries, you want a brace with medial and lateral hinges that control anterior-posterior motion and rotation. Post-surgical ACL braces are typically more restrictive early in recovery — limiting range of motion to protect the graft — and then transition to functional braces that allow normal movement while still preventing the harmful extremes.
For return to sport, a functional ACL brace like those from DonJoy or Össur's CTI line provides the kind of dynamic support that lets athletes train and compete with confidence. These aren't placebo braces — they're engineered specifically to offload the ACL during high-stress movements.
MCL Injuries
The MCL is on the inside of the knee and resists inward (valgus) force — the kind that happens when your knee collapses inward during a tackle or a bad landing. MCL injuries are graded 1 through 3 based on severity, and most Grade 1 and 2 injuries heal without surgery given proper rest, physiotherapy, and support.
A hinged knee brace that limits valgus stress is the right tool here. The hinge placement and rigidity matter — softer wrap braces don't provide enough medial support for a meaningful MCL sprain. You want something with solid polycentric hinges and a fit that holds the knee in a controlled position while it heals.
Most MCL patients are back to sport within 6 to 12 weeks with proper management. The brace is part of what makes that timeline realistic.
LCL and Posterolateral Corner Injuries
LCL injuries are less common than ACL or MCL injuries, but they tend to be more complex — partly because the lateral side of the knee involves not just the LCL but a network of posterolateral structures. These injuries often happen from a direct blow to the inside of the knee or in high-energy trauma.
LCL injuries require a brace with lateral support, and complex posterolateral injuries may need custom or semi-custom bracing to adequately control the instability. If you've been told you have a posterolateral corner injury, it's worth talking to someone who specializes in bracing before ordering anything online — the fit and design requirements are specific.
PCL Injuries
PCL injuries — less common than ACL but not rare — involve the ligament that prevents the tibia from sliding backward. They often happen in dashboard injuries or direct blows to a bent knee. Isolated PCL injuries frequently respond well to conservative management including physiotherapy and bracing.
PCL braces are designed to resist posterior tibial translation and are quite different in their mechanics from ACL braces. If you've been diagnosed with a PCL injury and you're shopping for a brace, make sure what you're looking at is actually designed for PCL — many braces marketed as "ligament braces" are primarily ACL-focused.
Post-Surgery vs. Return-to-Sport Bracing
These are two different stages with different requirements. Post-operative braces are designed to protect the repair or graft by limiting range of motion — they're hinged, they lock at specific angles, and they're often bulkier. Return-to-sport braces are designed for function — they need to be low-profile enough to move well in, durable enough to handle sport loads, and supportive enough to actually make a difference.
The mistake people make is using a post-op brace for return to sport (too restrictive, too heavy) or using a functional brace immediately post-surgery (not enough protection). The transition between them should be guided by your surgeon or physiotherapist — but knowing that the two types exist helps you have a better conversation with them.
Getting the Right Fit
Ligament braces need to be fitted carefully. A brace that migrates down the leg during activity, that applies pressure in the wrong location, or that doesn't correctly control the intended plane of motion isn't doing its job. For sport use especially, a proper fitting — where someone checks your leg measurements, watches how the brace sits when you bend and straighten, and confirms the hinge alignment — is worth the extra effort.
